We are planning to put together a Family Tree Book, and are asking for some input from each of you. This family history information would be gathered, organized and be used to put in the booklet.
We would like to try to trace the family history back as far as we can, so please be as informative as you can. Also pictures of past ancestors along with present family photo will be appreciated.
We are asking each family to donate $__________________ to cover the cost of having the book designed , organized, typed and printed. A front cover will be designed especially for the family name, and will be printed in color.
It is our hope that we can share more family history with everyone and know you will enjoy the outcome as much as the family that is putting it all together. Please return the forms in the enclosed envelope to:
